 Your GP will shortly be required to supply your personal and confidential medical information to the Health and Social Care Information Centre and NHS England.  The practice has NO CHOICE but to allow them to extract this information.  However individual patients can instruct their practice to stop the transfer of their data.  For more information on this important issue, please ask reception for a leaflet or see the link on the right of this screen, "Extraction of Personal Data". Below is an opt out form for completion if you do not want your information extracted. 




 If you want to register for this you will need to come to the surgery with two forms of identification.  One must be photographic, i.e. passport, driving licence, bus pass, and one must contain evidence of your address i.e. utility bill.  We will then give you log in details so that you are able to make telephone appointments with your doctor and you will be able to order your repeat prescriptions.

  This is not available to anyone under the age of 16 years.

The repeat prescriptions do actually link into your medical records so will be accurate. 

Patient Participation Group

The Practice recruits interested patients of this Practice to join its Patient Participation Group (or PPG for short). With the ever increasing need to engage with patients about primary care, PPGs can provide a means of meeting this need. It also enables practices to engage with patients on a range of issues and to gain their support with a variety of initiatives they may wish to undertake within a practice. PPGs provide patient feedback which helps practices with their decision making.

Please contact the Site Manager if you are interested in coming to a meeting or have any questions about it.

The practice has a group to provide a forum for patient input to the doctors. The aim of the group is to provide a patient's perspective on how the surgery operates. The group aims to meet every couple of months.

This input is both useful and important to the surgery.We are pleased to hear from our patients about the services we offer, and we are keen to welcome new members to the group.

We also run a virtual patient participation group which has over 600 members.  We email patients who have given their consent to be contacted by email, and in this way we can survey then their opinion.  This is a good, quick and efficient way to get a response from our patients.  Please tell reception if you would like to be involved in this way.
