 IMPORTANT CHANGES TO HOW WE HANDLE YOUR DATA 


 Your GP will shortly be required to supply your personal and confidential medical information to the Health and Social Care Information Centre and NHS England.  The practice has NO CHOICE but to allow them to extract this information.  However individual patients can instruct their practice to stop the transfer of their data.  For more information on this important issue, please ask reception for a leaflet or see the link on the right of this screen, "Extraction of Personal Data". Below is an opt out form for completion if you do not want your information extracted.

 DO YOU WANT TO REGISTER FOR ONLINE ACCESS TO MAKE TELEPHONE APPOINTMENTS AND ORDER REPEAT PRESCRIPTIONS?

 If you want to register for this you will need to come to the surgery with two forms of identification.  One must be photographic, i.e. passport, driving licence, bus pass, and one must contain evidence of your address i.e. utility bill.  We will then give you log in details so that you are able to make telephone appointments with your doctor and you will be able to order your repeat prescriptions.


  This is not available to anyone under the age of 16 years.


The repeat prescriptions do actually link into your medical records so will be accurate.

48 Hours Notice

This time is required to allow the request to go to the usual doctor so that (s)he can check the request against the medical record and ensure it is appropriate clinically appropriate, and safe, to issue further prescriptions.  This is important as we look after patients with very complex medication regimes.  In addition , medications can interact with other medicines so careful checks must be made. 

We also have to check that the patient’s condition has not changed since they were last prescribed the medication. furthermore prescribing advice might have changed since a patient was last issued medication, which means changes need to be made.

 All these checks take time, and this is why we need 48 hours to pepare your prescription.

You may telephone the repeat prescription line on Stourport (01299) 871699 between the hours of 10.00am to 12.30pm and 1.30 to 4.00pm.

Prescriptions for collection in the afternoon will be ready after 2.30 pm.  You may opt for the Pharmacy to collect your prescription direct from us and you collect the medication from the Pharmacy.  If you wish this then please let us know.  You can nominate a pharmacy to receive your prescriptions electronically.  Please contact your preferred pharmacy for this to happen.
