If you require any vaccinations relating to foreign travel you need to complete a travel risk assessment form. There are links below, or you may come to the surgery to collect a travel pack. The pack includes information on travelling safely abroad as well as any fees associated with travel. You should complete the form with the information required then return it to the surgery. The information you give will enable the Practice Nurse in the following week to work out the vaccinations (if any) you need and how long an appointment you will need. You should then ring the surgery about a week after handing in your form, if you have not heard from us, in order to book a travel appointment with the nurse.

Please allow plenty of time, preferably a couple of months, to complete this process These vaccines may have to be ordered if they are not a stock vaccine. You may need a course of treatment and you have to allow time for the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
